Community Choice Guide

The Community Choice Guide Program (CCG) is funded by Washinton State Medicaid and partners with our local Area Agency on Aging (AAA), Northwest Regional Council (NWRC). The program assists people transition into or maintain a healthy home for the participant to live as independently as possible.

 

This service comes to you in the community or at your home and is directed by you and your care plan. Areas that can be assisted include:

  • Filling out forms or applications and finding helpful community resources. 

  • Arranging physical moves and ensuring medical resources are available and met.

  • Setting up less formal or atypical daily activities.

  • Orient clients to new settings such as bus routes, grocery shopping, library visits, senior center activities, etc.

  • Assist with finding/relocating to another community setting, if needed for stabilization.

  • Take clients to the Social Security office to update the status of their benefits.

  • Shop for, pay and deliver household items.

  • Coordinate with client/family, community partners, care staff, housing, medical and legal personnel, caregivers, and other supports.

  • Assist in obtaining ID and filling out applications.

  • Attend Care Conferences in skilled nursing facilities or hospitals.

  • Visit and research housing options with/for the client.

  • Assist with moving arrangements.

  • Arranging for necessary relocation expenses; examples of expenses for qualifying individuals can include…

— Fees or deposits for utilities or services necessary for an individual’s health and safety such as pest eradication and one-time cleaning/allergen control.

— First, last, and Security deposits.

— Household furnishings.

— Moving expenses, and more.
