


Foundational Community Supports
Program
We currently are unable to provide this service at this time.
The housing and employment Foundational Community Supports programs is part of Washington State Medicaid, known as Apple Health, and can be activated so one can be provided free Case Managment support. Individuals must meet certain requirements in order to get this free service, so please note the bullet points below.

Must be age 16 or older.

Must be on Medicaid (if you are not, we will assist you get on Medicaid).

Must be presently homeless or recent history of homelessness, OR have a documented substance use disorder, OR have documented mental health disorder to receive housing services. (Expect Abundance will assist getting any necessary documentation.)

Client must also meet one of the following: have had 2 or more stays at a hospital, treatment center, or detox center over the last year, OR have frequent turn-over of care givers, OR have a PRISM score of 1.5+.
This service is meant to break and minimize barriers to our community members, so we meet a participant in their home or at a convenient location for them in the community to ensure safety for the participant and the case manager. Your case manager will meet with you as often as you need and depending on your goals. You, the recipient, will inform your case manager what you want to achieve, and together you two will make a plan and steps to reach and maintain those goals.
Supportive Housing Program
Supportive housing case management through Expect Abundance, is led by you and supported by me. We will sit together and create a plan to help you reach your housing goals, discussing together the elements that you want and need in your life to help you reach them, feeling fulfilled as you do.
Generally, the timeline to get a participant into housing will take 2-5 months, depending on what resources they already have, what resources we need to obtain together, and what housing is available at the price-point they need. This program is meant to help you build a foundation to keep growing off of and expanding as your life grows and expands.
These services are offered to you at your home, shelter, treatment center, or a public place that is convenient for you.

Supportive Employment and Education Programs
Supportive employment case management through Expect Abundance, is led by you and supported by me. We will sit together and create a plan to help you reach your educational or employment goals, discussing together the elements that you want and need in your life to help you reach them, feeling fulfilled as you do.
These services are offered to you at your home, shelter, treatment center, job/school or a public place that is convenient for you.
Together, we will make a plan towards obtaining and keeping employment that fits your schedule, your abilities, and your goals. With Expect Abundance, this program can support you in various ways including:

Resume/cover letter building

Interviewing skills

Finding affordable childcare

On-job skill-building and job coaching

Connecting to DVR and other state programs

Employee rights through the Disability Act

Working with employer or school to create accommodations

Obtain GED, certifications, AA/AS, BA/BS, Masters and PHD

Obtain specialty licenses

Connect with healthcare, mental health, and substance use services

Do I qualify?
Participant must be 16 years or older. You will need to have active Medicaid and not be in a spenddown. If you are unsure if you have either of these, we will assist finding out. This program is meant to support individuals who have chronic health needs or unmet conditions who need additional support finding and maintaining housing and/or employment. The conditions would need to have been documented in some way and are as follows: homeless (a self-statement is all that is needed), substance use disorder (SUD), or a mental health condition. Secondly, a person must have history within the last year of showing need of support by having three or more care givers, a high PRISM score, or two or more stays at a hospital, nursing home or facility, jail, treatment, detox or crisis center.
How does FCS support an already established organization?
If you are an organization or facility that serves people who are chronically homeless, struggle with substance use or mental health conditions, likely you are working with individuals who would qualify for this free case management service. Expect Abundance can meet with individuals identified through you needing support or can schedule visits to your facility to screen people to see if they qualify and then enroll them in the program. Weekly or as often as needed, case management can be brought to your facility to meet with enrollees to plan their next steps on their path towards stability before they leave your center, setting them up for a much more successful outcome and drastically lowering the likelihood of them returning to past habits. We will explore and set-up resources available to them succeed, conduct housing and/or employment search and apply to places when appropriate, and plan for ways that will help them stay on the path that they want for themselves.
